We are seeking an experienced Senior Care Coordinator to join our Home Healthcare Team at ANG MO KIO - THYE HUA KWAN HOSPITAL LTD.
Estimated Salary: SGD 60,000 - SGD 80,000 per annumJob DescriptionAs a Senior Care Coordinator, you will play a vital role in providing holistic support services to clients and their caregivers. Your key responsibilities will include:
- Assessment and Referral: Conduct thorough assessments of potential clients and their home environments to identify their needs and develop personalized care plans.
- Care Plan Development: Collaborate with caregivers and family members to create individualized care plans that cater to clients' unique requirements.
- Ongoing Support: Monitor clients' conditions, review and update care plans as needed, and provide ongoing support to ensure seamless transitions throughout the care journey.
- Financial Counselling: Offer financial guidance and advice on government subsidies to help clients navigate the complexities of eldercare financing.
- Payment and Administration: Collect payments and issue receipts for services delivered, ensuring smooth administrative processes.
To be successful in this role, you must possess:
- A minimum of a Diploma/Bachelor's Degree in Nursing or equivalent qualification.
- Registration with the Singapore Nursing Board.
- At least 3 years of working experience in a related field, with preference given to those with experience in Home Care services.
