Job Title: Skilled Nurse Specialist

Role Overview

We are seeking a highly skilled and compassionate Nurse Specialist to join our team. As a key member of our healthcare organization, you will provide high-quality patient care in a home setting.

Key Responsibilities:
  • Deliver safe and effective nursing care to patients in their own homes, ensuring seamless continuity of care after hospital discharge.
  • Independently execute complex nursing procedures under the Hospital at Home programme, including adult and pediatric population management.
  • Conduct thorough patient assessments, provide appropriate interventions, and evaluate outcomes, escalating required situations to doctors as necessary.
  • Effectively communicate patient information to internal and external healthcare teams, providing accurate and timely updates via suitable channels.
  • Participate in handover meetings with partner hospitals to ensure smooth transitions of care.
  • Provide caregiver training for patients and family members as required, enabling them to provide adequate care upon discharge.
Requirements:
  • Valid SNB registration as a Registered Nurse.
  • BCLS + AED certification preferred.
  • Hospital experience is desirable; fresh graduates will be considered and provided with adequate training.
  • Ability to work under pressure and manage situations effectively and independently.
  • Team player with good initiative and strong client-focused attitude.
  • 5-day work week with shift hours and weekend commitments.
  • Comfortable visiting patients' homes and performing nursing duties in a home setting.
