Transition Support Officer Role

This position is ideal for individuals with excellent interpersonal skills, strong organisational abilities and a passion for working with people with intellectual disabilities.

The Transition Support Officer will provide hands-on support, training and guidance to clients during significant life transitions. This may involve assisting clients in transitioning from School to Training & Development Centre, or from residential to community care.

  • Main Responsibilities:
  • Support Clients in Achieving Personalised Goals
  • Collaborate with Social Workers to Engage Clients and Caregivers in Discussion on Transitions and Training Needs
  • Coordinate and Participate in Transition Planning Meetings, Case Reviews and Individual Development Plan Meetings
  • Conduct Re-assessment of Clients' Transition and Development Needs as Necessary
  • Document and Keep Records of Outcomes of Clients' Personalised Plans

The ideal candidate should possess a strong passion for empowering individuals with intellectual disabilities and have exceptional communication and problem-solving skills.
