We are seeking an experienced Shared Care Initiatives Manager to join our team. In this role, you will be responsible for overseeing the development, implementation, and management of shared care initiatives within our organization. This involves strategic planning and development of long-term goals for shared care initiatives and aligning them with our overall mission and objectives.
Key responsibilities include coordinating care between primary care providers, specialists, and other healthcare professionals to ensure integrated and person-centered care. You will work closely with multidisciplinary teams to enhance collaboration, improve patient outcomes, and streamline care processes to ensure seamless patient transition to the community.
The ideal candidate will have a Bachelor's degree in healthcare administration or a related field, as well as a minimum of 10 years of experience in healthcare management, care coordination, or a related role. Strong analytical, leadership, administrative, and interpersonal skills are required, as is the ability to articulate ideas, plan, and strategize well. Additionally, you should be committed to continuing process improvement and have problem-solving and critical and creative thinking skills.
