Job Title: Elder Care Program Manager

About the Role:
The Elder Care Program Manager plays a vital role in the delivery of care services within an assigned cluster. This position is responsible for client intake, data management, volunteer coordination, and supporting the execution of community outreach activities.

Key Responsibilities:
  • Takes ownership of assigned cluster operations, ensuring all activities and services are delivered smoothly and efficiently.
  • Maintains up-to-date records with no lapses to support reporting and follow-up actions.
  • Conducts intakes, assessments, and ensures complete and accurate data entry in the system.
  • Serves as the primary point of contact for care beneficiaries within the cluster.
  • Coordinates monthly distributions (e.g. meal, grocery packs) according to the needs of the cluster.
  • Facilitates and supports events and initiatives that promote care and wellness.
  • Supports the planning and execution of monthly events and community outreach activities.
  • Liaises with and coordinates volunteers for befriending programs and community visits.
  • Ensures proper matching of volunteers and supports volunteer management efforts.
  • Participates in hotline duty rotation, responding promptly and appropriately to calls.

Requirements:
  • Minimum Diploma in Social Work or related disciplines.
  • At 2 years of relevant experience.
  • Experience in community engagement or care services is preferred.
  • Strong interpersonal and communication skills.
  • Detail-oriented with good organizational and data management abilities.
  • Able to work independently and collaboratively in a dynamic team.
  • Willingness to work occasional evenings or weekends for events or urgent support.
  • Possesses a good command of spoken and written English and Mandarin to liaise with Mandarin speaker recipients.
