Community Care Professional

  • As a Community Care Professional, you will be responsible for providing comprehensive patient care management for elderly individuals in the community. This includes conducting assessments, physical examinations, developing care plans, and performing clinical procedures as needed.

  • You will deliver patient and caregiver education on health promotion, discharge teaching, and training on care techniques to support independent living.

  • Uphold infection control standards through practice, monitoring, and public education to ensure a safe environment for patients and staff.

  • Guide and support junior nursing staff and care team members in delivering high-quality care and achieving excellent patient outcomes.

  • Maintain professional development through ongoing training, supervision, and participation in learning activities to enhance your skills and knowledge.

  • A diploma or degree in Nursing is required, with registration with the Singapore Nursing Board.

  • An Advance Diploma in Geriatrics, Medical-Surgical, Chronic Disease Management, or Community Health is highly advantageous.

  • At least 2 years' experience in nursing is necessary, with strong knowledge and understanding of community care practices.
