Job Title

We are seeking a skilled and dedicated Programme Coordinator to join our team. This is an exciting opportunity for a professional who is passionate about delivering high-quality services and making a positive impact in the community.

The Programme Coordinator will play a crucial role in facilitating the implementation of our programme and supporting operations under the leadership of Programme Clinical Leads. Key responsibilities will include coordinating, implementing, and evaluating works related to the programme, as well as assisting in preparing and setting up the programme framework.

Main Responsibilities:

  • Screen referrals for programme eligibility and direct patients to service tiers that best match their needs.
  • Engage eligible patients in enrolling into the programme.
  • Collect data on patient progress and inform referring clinicians about follow-up issues.
  • Support patients in navigating the programme and redirect them to suitable service tiers if they clinically deteriorate.
  • Maintain relationships with key agencies and community partners, such as Day Rehabilitation Centres.
  • Implement workflows for seamless referral and operations.
  • Contribute to enhancing service quality and the programme's component services.
  • Coordinate project team discussions and stakeholders' engagements.
  • Serve as the Point-Of-Contact and clarify programme-related matters when required.
  • Track programme indicators and provide progress updates/reports to stakeholders.

Requirements:

  • Independent with good initiative.
  • Excellent interpersonal and communication skills.
  • Meticulous and organized.
  • Proficient in Microsoft Office applications (Word, Excel, PowerPoint).
  • Relevant working experience in acute/intermediate and long-term healthcare is advantageous.
