About The Role
This challenging and rewarding role involves working closely with internal stakeholders to ensure seamless service delivery, managing inquiries related to healthcare services, coordinating requested services, and assessing customer needs using screening tools. The ideal candidate will also collaborate with health divisions and external partners on service referrals, submit referral forms, and follow up on action points.
Key Responsibilities:
- Collaborate with various teams to facilitate smooth service delivery
- Manage customer inquiries and coordinate relevant services
- Assess customer needs and recommend suitable healthcare packages
- Collaborate with health divisions and external partners on service referrals
- Conduct regular home visits and telephone follow-ups to engage with clients
- Counsel and educate caregivers on providing effective care and support
- Work closely with operations and nurse manager on client discharge from services
- Participate in program development and evaluation to enhance service offerings
Requirements:
To be successful in this role, you should possess strong communication skills, attention to detail, and the ability to work effectively in a team environment. A higher diploma or degree in any field is preferred, as well as experience in a similar role or industry.
