As a Community Care Specialist, you will conduct home visits to provide psycho-education, support, and care coordination for clients and caregivers. You will also assess patients' medical, psychological, social, environmental, and vocational needs.
Key Responsibilities:
- Conduct home visits to provide psycho-education, support, and care coordination for clients and caregivers
- Assess patients' medical, psychological, social, environmental, and vocational needs
- Collaborate with Allied Health Professionals, doctors, and nurses to ensure holistic care
- Offer psychological expertise in multi-disciplinary meetings
- Provide training for team members and caregivers
- Facilitate Mental Health support groups for patients and caregivers
- Execute projects and responsibilities assigned by the supervisor or Head of Department
- Registered Nurse with the Singapore Nursing Board
- RMN/Advanced Diploma in Mental Health or relevant qualifications (e.g., social work, counseling, psychology)
- 3 to 5 years of psychiatric experience (Community psychiatric clinic/IMH preferred)
- Skilled in psychotherapy for mental health patients
- Strong organizational, interpersonal, communication, and writing skills
- Proficient in MS Office and familiar with healthcare environments
- Ability to multi-task, work independently, and collaborate effectively
- Passionate about supporting individuals with mental health disorders
