Medical Social Care Specialist

This role involves providing psychosocial care to patients and their families across various community settings. As a specialist in this field, you will work closely with healthcare professionals to develop individualized care plans and facilitate family conferences for effective communication and shared decision-making.

  • You will conduct comprehensive assessments of patients' and caregivers' needs, providing emotional support, counseling, and psychoeducation to facilitate coping with illness, grief, loss, and life transitions.
  • Act as an advocate and liaison for patients and their families, ensuring access to necessary resources and services.
  • Maintain accurate and timely documentation of assessments, interventions, and progress notes according to professional standards and organizational policies.
  • Collaborate with multidisciplinary teams to enhance patient outcomes and service delivery.
Requirements
  • A Master's or Bachelor's degree in Social Work is required for this role.
  • Registration with the relevant professional body is preferred.
  • Excellent interpersonal skills, with the ability to communicate effectively with patients, families, and healthcare professionals are essential.
  • At least 3 years of relevant working experience in healthcare or eldercare settings is required.
