Job Title: Social Support Professional

Social Workers play a vital role in supporting vulnerable individuals and families. As a Senior Social Support Professional, you will work closely with clients to assess their needs, develop individual care plans, and provide therapeutic support.

The ideal candidate should have a Degree in Social Work, 1-2 years of experience in social work, preferably in residential settings for the elderly. Strong interpersonal and communication skills are essential for building rapport with residents and their families.

You must be able to coordinate with external agencies, such as hospitals and polyclinics, to secure financial assistance for medical appointments. The ability to evaluate existing, new, and prospective clients based on their needs, desires, and limitations is crucial.

Casework evaluation, maintaining case notes/documents, and updating clients' particulars and social reports/reviews are also key responsibilities.

A risk assessment background and experience with financial assessments and fund management are desirable. This role requires strong organizational and time-management skills, as well as the ability to prioritize tasks effectively.
