Job Description:

As a Clinic Care Coordinator, you will play a vital role in coordinating and providing patient care activities. Your responsibilities will include assisting in tracking, performing, and documenting established clinical indicators and care given to patients.

Our healthcare institution is looking for a dedicated and passionate individual who is willing to learn and explore the healthcare industry. If you have relevant healthcare experience, it will be advantageous. However, if you are eager to develop your skills and knowledge, we welcome your application.

Key Responsibilities:

  • Assisting in coordinating and providing patient care activities.
  • To use IT systems to track, perform and document established clinical indicators and care given to patients.
  • Perform basic parameter measurements, close preventive screening and vaccination gaps in relation to national programmes/initiatives.
  • Provide basic health education to improve patients' knowledge on lifestyle management, social and physical activities through social prescribing, the use of health monitoring devices e.g. home blood pressure, glucose and weight monitoring, as well as advise patients on vaccination requirements.

Requirements:

  • Diploma in Health Promotion will be an advantage.
  • Prior healthcare experience will be advantageous.
  • Good verbal and written communication skills.
  • A team player with good interpersonal & communication skills.
  • Passionate to work in the Healthcare industry.
  • Customer-oriented, a team player, a fast learner and serious with work.
