We are seeking a skilled Mental Health Case Manager to join our team at FILOS COMMUNITY SERVICES LTD. This role is perfect for an individual who is passionate about providing high-quality care and support to individuals with mental health challenges.
About the Role
The successful candidate will be responsible for providing case management, psychosocial intervention, care coordination, and recovery support to clients with alleged or diagnosed mental health issues. You will work independently to conceptualize, implement intervention plans, and follow up with clients as needed. Additionally, you will provide intake and referral services, collaborate with community partners and stakeholders for complex cases, and facilitate groupwork and program development when required.
Requirements
To be successful in this role, you should have a Grad Diploma/ Degree in Social Work and experience in working with clients on Mental Health challenges and Person-Centred Care and recovery. You should also possess good organizational skills, a strong service mindset, ability to work independently and as part of a team, and proficiency in MS Office. Furthermore, you should be able to travel and conduct home visits to different locations.
Benefits
