Job Title: Care Coordinator

Job Summary:

We are seeking a compassionate and skilled care coordinator to join our team. In this role, you will provide high-quality patient care and support to residents in a geriatric setting.

Responsibilities:

Clinical Nursing Quality and Safety:

  • Collaborate with healthcare professionals to ensure consistent and excellent clinical and service quality for residents and sponsors.
  • Work closely with senior staff nurses and other junior staff to achieve four zeros - zero falls, zero medication errors, zero nursing home-acquired pressure ulcers, and zero nursing home-acquired infections.

Clinical Care:

  • Assist in administering medications and performing simple dressings after passing competency in medication administration and wound dressing.
  • Monitor vital signs and assist in admission, assessment, and orientation of new residents.

Supervision:

  • Provide supervision to newly recruited enrolled nurses and junior staff, ensuring they perform duties according to standard operating procedures.
  • Oversee the serving of meals and feeding of residents under the supervision of care staff.

Residents' Activities:

  • Conduct daily activities for residents and maintain accurate logbooks.
  • Support physiotherapists and therapist assistants in rehabilitation programs and encourage resident participation.
  • Perform motion exercises and assist residents in walking.

Resource Management:

  • Optimize food costs by minimizing waste and promoting efficient use of consumables, supplies, dressing materials, and diapers.

Feedback Management:

  • Investigate and resolve all feedback and complaints received from centers.

Other Duties:

  • Assume additional responsibilities assigned by senior center managers or other authorized personnel.
  • Maintain an accurate logbook on daily activities in the center.
  • Communicate effectively with residents and sponsors.

Qualifications:

  • Locally trained: NITEC in Nursing
  • Foreign-trained: Degree or Diploma in Nursing with Board Pass (Professional Regulation Council)
  • State Enrolled Nurse with valid practicing license issued by the Singapore Nursing Board
  • Basic Cardiac Life Support certification
  • At least 1-2 years of post-registration experience in a geriatric setting is preferred
  • Basic Nursing Competencies
