Job Description:

Population health management is a critical component of healthcare planning, involving the analysis and interpretation of data to inform strategic decision-making. As an Executive Director of Healthcare Planning, you will play a key role in driving population profiling to support informed system planning and care delivery.

The ideal candidate will possess strong analytical and communication skills, with the ability to consolidate and report on population health indicators performance. They will also be responsible for facilitating strategic planning and coordination for population health initiatives, as well as supporting data and analytics requests for population health projects.

Key Responsibilities:

  • Develop and maintain population health profiling products to support informed decision-making.
  • Consolidate and report on population health indicators performance to track progress and identify areas for improvement.
  • Facilitate strategic planning and coordination for population health initiatives.
  • Support data and analytics requests for population health projects.

Requirements:

  • A bachelor's degree in Mathematics, Public Health, or Statistics.
  • Experience in population health or healthcare planning roles.
  • Working knowledge of Microsoft Office required; experience with data analytics and visualization tools an advantage.
  • Strong written, analytical, and communication skills.
  • Good people management skills and ability to manage multiple projects effectively.
  • Ability to work independently and as part of a team.
