Job Title: High-Quality Patient Care Coordinator
Job Description:
We are seeking an experienced and dedicated Patient Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care through effective assessment, planning, implementation, and evaluation of nursing care.
Roles & Responsibilities:
- Provide cost-effective care in accordance with established policies and procedures, standards of nursing care, and practices.
- Assess the needs and delivery of quality geriatric/dementia nursing care to clients.
- Perform pre-assessment of new clients referred by AIC.
- Ensure client enrollment complies with the company's SOP.
- Administer documentation to be submitted to relevant government bodies.
- Coordinate with the healthcare team to assess, plan, implement, and evaluate client conditions.
- Monitor, assess, and review clients' progression and implement care plans.
- Provide health promotion and counseling to clients and families.
- Consult and coordinate with the healthcare team to assess, plan, implement, and evaluate client conditions.
- Perform nursing care such as medication administration, wound dressing, and other technical skills as required.
- Oversee and manage the performances of the nursing team/care staff to ensure desired work competency levels.
- Coordinating, implementing, and monitoring clinical and service quality performance indicators for continuous improvement plans.
- Perform CPR, AED, and first aid for clients and actively participate in emergency situations.
Secondary Responsibilities and Duties (20%):
- Participate in clinical service projects.
- Participate in infection control campaign activities.
- Assume additional responsibilities as assigned by Senior Nurse Manager.
- Participate in training and development of care staff.
Requirements:
- Minimum diploma in nursing.
- Minimum 5 years acute ward experience.
- Valid SNB and BCLS certifications.
