Role Summary:

We are seeking an experienced and compassionate Care Coordinator to join our team. The ideal candidate will have a strong background in social work, with excellent communication and interpersonal skills.

Job Responsibilities:

  • Create personalized care plans tailored to the needs of residents, ensuring their physical, emotional, and psychological well-being.
  • Implement care plans and regularly record residents' progress, making adjustments as necessary to ensure optimal outcomes.
  • Design and organize programs and activities to address residents' care requirements, promoting engagement and participation.
  • Prepare and submit review reports and monthly interview records every six months, providing valuable insights into residents' progress.
  • Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents, including healthcare professionals, family members, and support staff.

Requirements:

  • A minimum diploma or degree in Social Work, Psychology, or Counselling.
  • At least one year of relevant experience in the healthcare industry, preferably in geriatric care.
  • Excellent interpersonal and problem-solving skills, with the ability to communicate effectively with residents, families, and healthcare professionals.
  • Proficient in English and able to converse in dialects or other languages, if required.
  • Able to work five days a week, with flexibility to adapt to changing circumstances.
  • Possesses a genuine passion for working in a geriatric care setting, with a commitment to delivering high-quality care.
