Job Title: Paediatrics Trained Home Care Nurses

Job Description:

We are seeking a skilled and experienced paediatrics trained home care nurse to join our team. As a senior/staff nurse, you will provide high-quality nursing care services to patients in their homes.

Key Responsibilities:
  • Deliver safe, competent, and individualized nursing care services that meet the needs of patients.
  • Manage complex patient care by demonstrating clinical skills, knowledge, and critical thinking in care delivery.
  • Monitor, reassess, and evaluate patient conditions; implement care plans and support families with advanced disease management.
  • Coordinate patient care with physicians, provide direct care, evaluate conditions, and liaise with families and healthcare professionals.
  • Build rapport with patients and families; assess home needs, and requisition equipment and aids.
  • Allocate necessary equipment to meet care requirements.
  • Follow up to ensure continuity of care and treatment plans.
  • Perform nursing procedures such as medication administration and wound dressing.
  • Maintain accurate documentation of patient conditions and related data.
  • Participate in multidisciplinary team meetings and collaborate for seamless patient care.
  • Ensure patient safety and observe legal storage and control of drugs and supplies.
  • Maintain confidentiality and discretion in patient matters.
  • Properly care for equipment and use supplies economically.
  • Handle public inquiries professionally.
  • Follow organizational policies, Singapore Nursing Board standards, and ethical guidelines.
  • Stay updated with industry guidelines and develop necessary skills.
  • Maintain high standards of housekeeping and workplace organization.
