We are seeking a Care Coordinator to join our Special Education Schools team.
In considering this role, we value professionals who are passionate about connecting with people of diverse abilities, uncovering possibilities, and harnessing the power of communities.
Job Responsibilities:
The Care Coordinator will work closely with families, social services, and community agencies to strengthen care networks around our clients with disabilities.
This includes conducting needs assessments, providing guidance and support, and participating in programme development.
We offer a competitive salary of $55,000 - $65,000 per year, depending on experience.
Requirements:
- Degree in Social Work
- Passion for supporting persons with disabilities and their families
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of a team
