Job Summary

We are seeking a skilled and compassionate Care Consultant to join our team at AGENCY FOR INTEGRATED CARE PTE. LTD.

Salary: $6,000 - $8,000 per month

About the Role

The successful candidate will be responsible for assessing client care needs in various settings, managing daily operations, and delivering high-quality customer service.

Key Responsibilities:

  • Conduct thorough assessments of client care needs in home, hospital, and institutional settings to facilitate referrals and placements in suitable services/care facilities.
  • Manage day-to-day operations to ensure prompt and quality customer service delivery.
  • Engage clients and caregivers in identifying care needs, formulating care plans, and linking clients to service provisions.
  • Assess and match relevant financial schemes, facilitating applications to support clients' care expenses.
  • Solicit feedback from clients/caregivers to review service standards and improve customer satisfaction.
  • Liaise with hospitals, referring entities, and service providers to facilitate client placements.
  • Participate in case discussions and client/family conferences with stakeholders.
  • Contribute to initiatives/projects/events and assist as a subject matter expert for continuous improvement.
  • Collate data, monitor trends, and identify possible service gaps.
  • Ensure proper documentation of all transactions into the IT system or case notes.
Requirements

To be successful in this role, you will need:

Professional Qualifications:

  • Professional qualification in Nursing or Allied Health disciplines relevant to the job.

Experience:

  • At least 3 years of experience in related fields in acute and/or community healthcare settings in Singapore.

Skills and Competencies:

  • Knowledge of geriatrics, community services, and financial schemes is an advantage.
  • Proficient in Microsoft Office (Word, Excel, PowerPoint).
  • Good problem-solving skills and attention to detail.
  • Pro-active work attitude with good interpersonal, communication, and organizational skills.
  • Able to work effectively and harmoniously with internal and external stakeholders.
  • Able to keep abreast of clinical assessment tools, community services, and schemes.
  • Able to work independently, multitask effectively under pressure, and adapt to changes.
  • Able to comply with process/manuals, advisories, guidelines, policies, legislations/Acts relevant to healthcare and the area of work.
