Job Title: Patient Service Associate (Community Nursing)
Job DescriptionWe are seeking a detail-oriented and organized Patient Service Associate to join our community nursing team. The ideal candidate will have excellent communication skills, be able to work independently, and have a passion for delivering high-quality patient care.
Key Responsibilities:- Manage inquiries on health services and coordinate requested services
- Assess customer needs using screening tools and recommend suitable health service packages
- Collaborate with health divisions and external partners on service referrals, submit referral forms, and ensure follow-up on action points
- Conduct regular home visits and telephone follow-ups to engage with clients
- Counsel and educate caregivers on providing effective care, support, and available financial assistance
- Work closely with the Operations and Nurse Manager on client discharge from services
- Participate in regular program development and evaluation to improve service offerings
To be successful in this role, you will need:
- A minimum higher nitec or diploma in any field
