Job Scope:

The successful candidate will be responsible for performing home visits to chronic ill/palliative patients who are being referred. They will perform nursing assessments on the patient, coordinate with doctors for medical assessment and follow-up visits, monitor current medication and compliance as prescribed by the doctor, and initiate nursing care plans.

Responsibilities:
  • Nursing procedures such as wound dressing, insertion of urine catheter, insertion of naso-gastric tube, and care of tubings as required
  • Education of caregivers/families on basic nursing procedures to ensure continuation of care at home
Requirements:
  • Minimum - 2 years experience in homecare
  • Able to start work immediately
