• Guiding patients to enrol through HealthHub app for patients with Singpass


• Onboarding patients to HealthierSG, including booking of appointments (doctor consultation and care coordinator appointments).


• Tagging patients on Queue Management System (QMS) for HealthierSG related visits


• Answering patients' queries with regards to HealthierSG
