Roles & Responsibilities

The Pharmacy Technician at our Tsao Foundation’s Primary Care Clinic (PCC) will play a crucial role in providing pharmaceutical support and enhancing the quality of healthcare for our elderly patients. This position encompasses a wide range of responsibilities, including medication packing, dispensing, education, and medication reconciliation. The Pharmacy Technician will collaborate within a multidisciplinary clinic, working closely with physicians, nurses, clinic administrators, and fellow healthcare professionals to ensure optimal patient care.

Medication Packing:

  • Prepare medications, unit dose packaging, or other specialized packaging such as pill boxing as needed to aid in medication adherence for geriatric patients.

Medication Dispensing:

  • Accurately prepare and dispense prescribed medications for geriatric patients in compliance with established safety and regulatory guidelines.
  • Review medication orders to identify any potential drug interactions or discrepancies.
  • Maintain proper storage and labeling of medications to ensure patient safety.

Drug Inventory Management:

  • Performs cycle count for stock accountability
  • Procure drugs according to inventory par levels
  • Monitor expiry dates, par levels and price changes

Medication Reconciliation:

  • Conduct medication reconciliation during patient appointments, ensuring that patients' medication lists are accurate and up to date.
  • Collaborate with other healthcare providers to resolve any discrepancies or concerns related to patient medications.

Patient Education:

  • Provide clear and understandable medication counselling to patients, caregivers, or family members to promote medication adherence and safe medication practices.
  • Address patient questions and concerns related to their medications and potential side effects.

Collaborative Healthcare Team Participation:

  • Work closely with physicians, nurses, pharmacist and clinic executives to enhance the quality of providing patient care.
  • Participate in multidisciplinary meetings and communicate effectively to ensure comprehensive care for geriatric patients and improve clinic operations.

Medication Safety:

  • Maintain awareness of medication recalls, updates, and safety alerts, taking prompt action to prevent patient exposure to unsafe medications and prevention of medication errors.
  • Maintain a culture of reporting medication errors
  • Follow strict protocols for medication storage, handling, and disposal.

Compliance and Documentation:

  • Ensure compliance with all relevant pharmacy laws, regulations, and clinic policies.
  • Maintain accurate and organized records of medication dispensing, pharmacy interventions, patient interactions, and inventory management.

Continuous Learning and Professional Development:

  • Stay updated on new medications, pharmaceutical technologies, and best practices in geriatric care.
  • Attend training and professional development opportunities to enhance skills and knowledge.

Requirement:

  • Minimum Diploma in Pharmaceutical Science and/or Certified Pharmacy Technician Certification or equivalent.
  • Minimum of 2 years of experience as a pharmacy technician, preferably in a clinical or geriatric setting.
  • Proficient in medication packaging and dispensing techniques.
  • Strong knowledge of medication safety, drug interactions, and pharmaceutical regulations in Singapore.
  • Excellent interpersonal and communication skills, especially in working with elderly patients.
  • Ability to work effectively in a collaborative healthcare team.
  • Detail-oriented with a commitment to accuracy and patient safety.
  • Willingness to adapt to a dynamic clinical environment.
