Roles & Responsibilities

Job Scope:

  • Collect basic socio-demographic information for patients assigned by reporting officer
  • Confirm with patients for their attendances of first visit / follow up appointment
  • Triage cases based on their clinical needs
  • Provide psycho-education for patients and their family members
  • Responsible for the arrangement of care plans for Psychogeriatric and Child/Adolescent patients
  • Administer brief psychogeriatric assessments and brief child assessment (Continuous Performance Testing) and collect basic socio-demographic information for patients
  • Obtain clinical information from patients via use of scales (ADHD, depression, anxiety rating scales) prior to patients’ first visits
  • Facilitate discharge to step-down care (polyclinics, GP clinics, SLH etc) once diagnosis and care plans are finalized
  • Perform ECG, blood taking, memory assessment and others (if any) for patients (training to be provided)
  • Perform data management, collating and maintaining of patient database for clinical and research purpose
  • Ad hoc duties as assigned

Job Requirements:

  • Degree in Nursing, Social Work, Psychology, Allied Health or equivalent
  • Good interpersonal and communication skills
  • Proficient in using MS office
  • Knowledge in geriatric, adolescent/child care and community skills will be an advantage
