JOB SUMMARY:

The Care Coordinator position entails overseeing, managing, and coordinating cases for the organization. The primary responsibility involves keeping the team informed about case details and collaborating with Care Recipients' families to facilitate effective information sharing. This role plays a direct role in supporting clients, providing them with resources and aid. Additionally, it necessitates strong multitasking abilities and exceptional negotiation skills.

DUTIES AND RESPONSIBILITIES:

  • Responding to inquiries and reaching out to potential clients through phone communication to understand their requirements and offer insights on iKare's products and services.
  • Managing and facilitating care for the Care Recipients comprehensively across assessment, evaluation, planning, and execution phases.
  • Establishing connections and collaborating closely with the Care Recipient's family, Physician, Nurses, or relevant Care team member to tailor personalized care plans and deliver top-notch services to the Care Recipients.
  • Scheduling and coordinating home visits/session arrangements for the Care team member/Nurse and Client.
  • Conveying the Care Recipient's needs and care details to the respective Care team member and ensuring appropriate follow-up actions.
  • Recording and managing assessments, plans, and results promptly and accurately, updating the relevant system and keeping the team informed.
  • Engaging in regular follow-ups with the Care team and Care Recipients' families to assess progress, ensure continuity of care, and enhance health outcomes.
  • Connecting the Care Recipients' families with social service programs and benefits like transportation aid, government support, and other related resources.
  • Managing clients' service agreements, payments, and invoices in collaboration with the Finance team.
  • Participating in and supporting company events, fairs, educational discussions, and training sessions.
  • Addressing any other assigned tasks as needed.
  • Maintaining high-quality service aligned with the Company's corporate standards

REQUIREMENTS:

Applicants should hold a degree in Psychology, counseling, social work, nursing, or a related field, accompanied by relevant professional experience.

Proficiency in PC applications (Microsoft Word & Excel) is necessary.

Strong interpersonal skills and written communication abilities are essential.

The ideal candidate is self-motivated, possesses excellent social capabilities, can network effectively with diverse professionals, and showcases problem-solving skills.

A team player with a genuine dedication to patient care, a willingness to learn, and the capacity to contribute positively to the team is preferred.

EXPERIENCE:

A minimum of 5 years of experience in healthcare, social services, or the hospitality sector is required.
