Job Summary
Purpose of job
The role involves proactively engaging clients, organising social activities and encouraging an active lifestyle to prevent social isolation. The position also entails referring clients to relevant community care and social assistance when needed. Under the Age Well SG initiatives, the role requires collaboration with primary care physicians (e.g., family doctors) to empower residents in taking charge of their journey toward healthier and more active lifestyles.
Key Responsibility Areas
- Plan and conduct evidence-based active ageing programmes.
- Plan and coordinate support for clients requiring befriending and buddying services.
- Lead group work and outreach activities.
- Support AAC-CREST integration initiatives.
- Recruit, train, and match volunteers to appropriate services.
- Build community partnerships and enhance existing resources.
- Administer new intakes and conduct necessary health assessments for clients.
- Monitor and record clients' vital signs.
- Maintain a membership and volunteer management database using the in-house AAC system.
- Perform administrative tasks, including maintaining up-to-date client and volunteer records.
- Attend to client inquiries.
- Continuously improve the quality of services and programmes.
- Undertake any other duties assigned by the Manager or Executive Director.
Requirements
Experience:
- Prior working experience in a Senior Activity Centre or Active Ageing Centre is advantageous.
- Resourceful, proactive, responsible, and client-focused.
- Strong communication and interpersonal skills; ability to converse in local dialects is an added advantage.
- Strong passion and commitment to working with seniors.
- Proficient in MS Office and Google Suite applications.
- Ability to work independently and as part of a team.
Qualifications:
Diploma in Social Work or relevant training certification.
