Summary
This position serves as a Telerheumatology Clinical Pharmacy Specialist (CPS) provider for the NTRP and is responsible for providing high quality comprehensive medication management for Veterans with rheumatic diseases/disorders. The Telerheumatology CPS functions at the highest level of clinical practice and works independently under their scope of practice to directly care for patients.
Duties
The Telerheumatology CPS is accountable to the Rheumatology Lead or one of the Deputy Directors. The Telerheumatology CPS will use professional judgment and expertise to make decisions about services and treatments provided to Veterans and their significant others/family members within his/her area of responsibility. His/her work is reviewed for technical adequacy and compliance with procedures and practices applicable to the subject. The Telerheumatology CPS will receive guidance and consultation on practice through VHA directives, handbooks, and information letters; VISN policies and procedures; and facility policy and procedures.Duties of this position include but are not limited to the following: Functions as an advanced practice provider with a scope of practice to provide CMM services for identified Spoke site facilities Core activities outlined within the scope of practice include: Executing therapeutic plans using the safest and most cost-effective medication treatments. Performing physical measurements and objective assessments necessary to ensure the patient's appropriate clinical responses to therapy. Ordering, subsequent review, and action on appropriate laboratory tests and other diagnostic studies necessary to monitor, support, and modify the patient's drug therapy. Ordering and prescribing medications, vaccines, devices, and supplies to include: initiation, continuation, discontinuation, monitoring, and altering therapy; prescribing, administering, or dispensing controlled substances may be included in the scope of practice only if the CPS is authorized by their State license to do so and comply with the limitations and restrictions on that authority Ordering consults and referrals, as appropriate, to maximize positive drug therapy outcomes; examples include, but are not limited to, referrals for routine care or assessment for additional care needs Obtaining and documenting informed consent for treatments and procedures that require consent for which the clinical pharmacist is responsible, including those where the clinical pharmacist is the prescriber of a treatment that requires consent or when they are providing medication management services on behalf of the original prescriber. Conducts Telehealth (CVT, VVC), Telephone, and in-person (when appropriate) encounters with patients for CMM in the Rheumatology practice area Follows-up with patients in a timely manner and monitors for drug therapy response, potential adverse drug reactions, allergies, compliance, laboratory results, and other monitoring measures as appropriate; treatment plan adjustments are made accordingly based on assessed measures. Review's patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, pharmacokinetic impact, and therapeutic outcomes as required; information is communicated with team members and appropriate alternatives to current treatment plans are provided Promotes and monitors compliance for established drug therapy policies Coordinates with and refers to appropriate diagnosing providers when disease progression or events occur that are outside the Telerheumatology CPS scope of practice Identifies patients who may benefit from CMM through population management, team meetings, and/or huddles Assures the safe, appropriate, cost-effective use of drugs in assigned patient care areas through the application of specialized knowledge and the use of judgment in the collection and interpretation of data specific to each patient and in tactful interactions with other professionals. Collaborates in the development and maintenance of policies and procedures defining operational requirements for the NTRP. Participates in performance improvement, peer review and quality improvement initiatives as necessary Attends and actively participates in NTRP, in-services, staff development, and relevant conferences Serves as an advocate for the pharmacy profession and educates team members on CRH and CPS provider processes Assists in the training for new NTRP staff, competency assessments, and mentoring as requested Performs other duties as assigned.
