Our halfway house accommodates 100 individuals undergoing community-based rehabilitation.
Our skills development initiatives seek to equip individuals for a successful reintegration into society.
Job Description
- Responsible for overseeing, coordinating, managing, and evaluating resident case management at Pertapis Halfway House (PHH) in alignment with the organization's strategies, policies, and directives from senior management.
- Supervise a team of case workers handling resident care coordination, develop and implement case management programs, and ensure compliance with regulatory standards and policies.
- Lead and mentor a team of case workers, identify opportunities for team training and skill enhancement, and conduct training sessions on laws, regulations, and policies.
- Supervise the planning, execution, and oversight of resident programming at the halfway house.
- Create and suggest relevant rehabilitation and reintegration programs to keep residents engaged during their stay.
- Analyze resident trends and emerging needs, and develop suitable intervention programs.
- Ensure program integrity for optimal outcomes, manage residents' safety, discipline, and progress during their stay.
- Maintain accurate records of admissions, discharges, personal belongings, and disciplinary meetings.
- Update and maintain all case management records and necessary documentation.
- Develop and review Standard Operating Procedures and Best Practices in line with the Halfway House Service Model and organization policies.
- Supervise Individual Care Plans for all residents and generate monthly statistical reports.
- Collaborate with the Education & Programs Department and stakeholders to meet residents' needs effectively.
- Execute any additional tasks delegated by senior management.
Job Requirements
- Degree in Social Work or Psychology OR Accredited Social Worker OR Accredited Social Service Practitioner
- Minimum 7 years of relevant experience
- Certification as a Substance Abuse Counsellor or Therapist is a plus
- Demonstrated supervisory and management skills
- Excellent interpersonal, verbal, and written communication abilities
- Capable of working with diverse age groups
