Job Description:

OBJECTIVES

  • S3 aims to provide the best quality of poststroke care in the community so that our clients and their families are able to rebuild their lives after stroke. The Wellness Team plays a critical role in this continuum of care by creating a safe and supportive environment for stroke survivors and their families to enhance their physical, social, and emotional health. And to curate and develop healthy ageing activities for Active Aging Centre (AAC) residents.
  • The Asst Manager/Manager will play a key role in overseeing and leading the development, evaluation and implementation of S3's Wellness Programme and AAC at Boon Lay.
  • He/she will lead the Wellness coordinators to operationalise the Wellness programme offerings, develop partnerships with service providers and partners, coordinate and plan programmes and provide a seamless onboarding experience for clients to the Wellness Programme.
  • He/she is essential in overseeing the Wellness Programmes and to ensure smooth daytoday implementation of administrative and operational duties in order to create the desired positive impact and success that our programme aims to achieve.
  • He/she will plan, budget and oversee the development of AAC programmes and activities for seniors and volunteer management programmes.

ROLES & RESPONSIBILITIES
Oversee Programme Management & Implementation

  • Plan and coordinate the schedule of both onsite and virtual Wellness sessions
  • Manage the recruitment and onboarding of wellness clients
  • Observe clients during sessions to ensure safety and wellbeing
  • Intervene to provide support to clients where necessary
  • Train and work with volunteers who can provide support during the Wellness sessions
  • Train and guide volunteers in implementing programmes as needed
  • Prepare documentation and reports as required
Programme Development and Evaluation

  • Evaluate, Curate, and Develop suitable activities for each pillar of Wellness and the Active Ageing Seniors e.g. Health, Socio-Emotional, Psychological, Nutrition
  • Adapt activities and programmes to the needs of clients and the environment
  • Evaluate programme efficacy and recommend areas of improvements that are sound and implementable.
Manage Service Providers and other stakeholders

  • Build a robust community ecosystem of care agencies, community partners and befrienders to support seniors and clients in the community
  • Oversee the execution of an outreach plan including followup visits, to engage with seniors residing in the designated blocks.
  • Manages communication and helps build positive relations within the team and external stakeholders

Oversee the following efforts by the coordinators:

  • Sources and liaise with service providers on services for clients and guardians
  • Acts as the point of contact for clients and guardians to provide advice, problem solve and facilitate the resolution to any issues
Administration Duties

  • Prepares paperwork including but not limited to onboarding recruitment, surveys, and programme reports
  • Assists to develop programme effectiveness surveys, compiles, and analyses the results and prepares reports as required
  • Any other ad hoc tasks as assigned

MINIMUM REQUIREMENTS

Academic Qualification

  • Minimum Degree holder

Languages

  • English and preferably at least one language

Experience

  • 58 experience, with preferred experience in a leadership role, or similar capacity, preferably from a VWO, \Non-Profit Organisation sector or in the Healthcare industry
