We are looking for Staff Nurse (Homecare ) responsible for providing high standard nursing care and management to Homecare clients.
Key Responsibilities:
1. Manage patient-centred care
a. Provide a comprehensive assessment of new referrals and accurately determine their healthcare needs (e.g. medical, nursing, psychosocial etc).
b. To develop, implement and review individual care plans after each assessment.
c. Perform basic health screening and monitor the health status of clients and make referrals as necessary.
d. Perform nursing care and procedures as per clients’ needs which includes:
· Wound care and management
· Change of and care of feeding tube, urinary catheter and other procedures
· Monitoring of blood pressure and blood glucose levels
· Medication management – packing and compliance monitoring
· Stoma care and management
e. Updating and discussing with appropriate health/ allied health professionals on client's needs and progress.
f. Provide health education for clients and caregivers.
g. Work with internal and external clients for all healthcare issues.
h. Maintenance of work environment and manage any safety or infection control issues as necessary.
i. Coordinate with other care disciplines/agencies for care integration and transition.
2. Engage patients in management of health conditions
a. Communicate and build rapport with clients and their families.
b. Administer self-management plans to empower clients, family members and caregivers to facilitate participation in their own health management
c. Implement individualised health education plan
3. Homecare Client Management
a. To assist the manager in planning and facilitating Homecare services for clients.
b. Work alongside the manager and team in the achievement of targets and management of outcomes set for Homecare periodically.
c. Maintain proper documentation of clients’ progress and accurate collation of data (record of referral / enquiry / attendance etc).
d. Preparation of data for monthly statistics and reports.
e. Manage client workload through effective planning and scheduling according to clients’ needs.
4. Provide emergency care
a. Participate in medical emergencies or crisis situations as appropriate.
b. Monitor for early signs and/or deterioration of client’s condition with appropriate physiological assessments and observations.
5. Others
a. Handle public enquiries of Homecare services.
b. Manage and monitor medical supplies, equipments and consumables inventory.
c. Participate in the training programme of various community partners for professional and personal development.
d. Liaise / collaborate with ComRel department in management and development of volunteers, facilitation and coordination of CIP from school / corporate groups, development and maintenance of corporate partnerships.
6. Requirements for the position:
- Registered Nurse with a Diploma in Nursing.
- Minimum 3 years of relevant nursing experience.
- Competent to independently provide nursing care according to protocols established.
- High integrity, resilient, passionate about nursing and helping others.
- Effective communications and interpersonal skills and able to communicate in dialects with non-English/Mandarin speaking clients.
- Strong team player, resourceful and systematic.
