Responsibilities- Assesses, plans, implements and evaluates clinical care for patients under his/her care.
- Performs clinic generic nursing duties like phlebotomy, injections and assisting in clinic procedures.
- Conducts pre- and post-procedure information, admission and discharge planning.
- Collaborates and cooperates with physicians and other health care personnel for a smooth delivery of patient care.
- Conducts diabetic eye and foot screening, escalates "at risk" cases to the Podiatrist based on protocol and in consultation with physicians.
- Provides nurse counselling, monitoring and patient education on chronic disease management and health promotion.
- Any other duties as delegated by the department reporting officer.
- Diploma / Degree in Nursing or equivalent with at least 2 years of clinical nursing experience.
- Registered with Singapore Nursing Board.
- Self-motivated and able to work well in a team.
- Good interpersonal and communication skills.
- Proficient with Microsoft Office Applications.
- Able to work in a dynamic environment as well as being flexible and open to changes.
