Job Description
Manage case referral, conduct assessment, planning, intervention and follow up on case referrals in CREST Team Provide casework intervention and counselling services, facilitate groups, conduct and implement structured programmes for the clients and their families. Conducts comprehensive client assessments to collect functional, environmental, psycho-social, financial, employment, housing, education and health information as appropriate to develop a creative and cost-effective case plan. Plan and conduct programmes and services that are relevant for this target group. Provide training for volunteers. Prepare statistical data with Senior Social Worker for submission to funders as required. Assist the Senior Social Worker in planning community outreach Adhere to high Professional Code of Ethics when performing case work. Perform other organizational duties as assigned by the Centre Director / Senior Social Worker Supervise Junior Case Workers Facilitate Support Group / Community Projects
