The Position
As the Clinical Pathway Consulting Partner, we are looking for an Advanced Practice Nurse, Clinical Pharmacist or individual with experience looking into the improvement of clinical pathways to be in a dynamic team that focuses on disease management and patient journey improvement, leveraging Roche’s high medical value diagnostic solutions. The candidate will be responsible for establishing consultancy expertise in clinical space in order to attain efficiency gains, optimized clinical practices and patient journey, and ultimately better patient management with better coordination across clinical stakeholders. He/She will establish this expertise, pilot use cases in focus disease areas in APAC, and contribute to the overall development of Healthcare Consulting.
Responsibilities:
Expand and establish Clinical Pathway Consulting strategy and framework that are relevant to current healthcare practices in APAC. Be the expert to build and strengthen this clinical pathway consulting expertise amongst APAC affiliates
Based on focus disease areas, eg cardiometabolic, oncology, infectious diseases,, pilot and execute consultancy projects in collaboration with affiliates, validate clinical consultancy concepts in various disease management pathways with the ultimate goal to improve patient management outcomes and change healthcare practices via the adoption of appropriate diagnostics tools and technologies.
Work collaboratively with doctors and other healthcare professionals to co-develop standardized care pathways and co-create new models of care to improve the management of patients with diseases such as heart failure, acute coronary syndrome, sepsis, breast cancer, liver cancer, lung cancer, and cervical cancer.
Incorporate data analysis, comparing different practices to identify gaps for improving patient care, co-create and deliver solutions to change clinical practices in order to demonstrate and achieve clinical efficiency and outcome, and create best practice examples.
Work with other regional teams, global, and local organizations and contribute to the overall development of Healthcare Consulting.
Qualifications:
Diploma/Bachelor degree in Nursing or Pharmacy from recognized training institutions as approved by the Singapore Nursing Board, Singapore Pharmacy Council or equivalent.
Registered Nurse or Clinical Pharmacist with at least 7 years of experience in healthcare management or operations in hospital and primary care, out of which at least 2 years must be in a hospital setting (inpatient, outpatient, or emergency).
Professional certifications in quality improvement such as Lean or Six Sigma are an added advantage
Experience in running outpatient or post-discharge clinics Heart Failure Clinic, Diabetes Clinic.
Possesses strong organizational skills in project management, communication, analytics and problem-solving, leadership, and interpersonal skills.
Strategic thinking and planning skills with proven analytical and forward-looking thought process
Stakeholder management skills with the experience of collaborations with cross-functional teams and different types of external stakeholders
Requirements:
Clinical practice experience and proven expertise in the following areas and specialties:
Disease Management: Manage chronic conditions and acute illnesses, prescribing and adjusting medications, treatment plans, and interventions. Educate patients on disease management and prevention.
Specialization: Provide specialized care, treatment, and expertise in cardiovascular, acute/critical care, and/or infectious diseases.
Prescriptive Authority: Able to prescribe medications and order/interpret blood-based diagnostics tests.
Clinical Assessment: Conduct comprehensive health assessments, including physical examinations, history taking, and diagnostic tests as well as interpret findings and provide accurate diagnoses.
Quality Improvement: Lead quality and continuous improvement initiatives that resulted in improved clinical, operational, and financial outcomes.
Collaboration: Work collaboratively with other healthcare professionals, including doctors, nurses, pharmacists, and laboratory technologists, to ensure holistic patient care.
