**THK**MC was incorporated on 13 October 2011 as a charity to provide multiple social and welfare services to the community at large, and achieved an Institute of Public Character (IPC) status on 18 November 2011. **THK**MC is the charitable arm of Thye Hua Kwan Moral Society, which lives by the mission To Serve Mankind to help anyone who needs help regardless of their race, religion, colour, language, creed and culture. **THK**MC today serves more than 70,000 beneficiaries through more than 70 programmes across five Services, which include Disability, Early Intervention for Children, Family, Seniors' Community, and Therapy.

DESIGNATION : Assistant, Client Care (Meals on Wheels / Medical Escort and/or Transport)

RESPONSIBILITIES
**Meals on Wheels**
- To deliver meals twice a day (lunch and dinner) every day to needy and ageing clients in their homes.
- Need to be able to differentiate Halal, Non-Halal (different plastic bag to differentiate between Halal and Non-Halal) and special request (Example: Cannot eat all types of beans, unit number of household will be written on the plastic bag).
- Plan and sort meals according to location.

**Medical Escort and/or Transport**
- Assist clients in boarding/alighting from the vehicle (bus) in line with clients' needs, operating wheelchair lift or other mobility devices where necessary.
- Administer safety procedures for clients before and during transportation to ensure safe and secure bus environment.
- Provide quality interaction/transport experiences to clients.
- Escort clients to and from the vehicle, clients' doorsteps, and/or throughout clients' appointments depending on services requirements.
- Provide escort and/or transport services for seniors from their residence to hospitals for medical appointments (i.e., setting of payment, scheduling of future appointments, collection of medicine at pharmacy).
- Promote safety and comfort for seniors by interacting with them while waiting for transport during appointments (i.e., handling wheelchair).
- Provide social monitoring and follow escalation protocol.
- Able to multi-task and is focused in coordinating the tasks on hand to ensure deliverables are met to client satisfaction.

**Data Management**
- Perform administrative tasks.

**Stakeholder Management**
- Communicate with internal / external stakeholders (i.e. various care teams in hospitals and supporting organization) on care information relating to wellbeing and care continuation of the seniors.
- Correspondences with clients etc.

**Asset Management**
- Responsible for needs of the clients.

**Learning & Growth**
- Basic Cardiac Life Support (BCLS)
- First Aid
- Food Hygiene

**Any other duties as assigned by Care Client Executive.**

QUALIFICATIONS
- GCE 'N'/'O' Level or Nitec/Higher Nitec
- Tech-Savvy (Computer/Tablet Literate)
- Ability to communicate in English and local languages.
- Good interpersonal and communication skills.
- Strong team player and pleasant personality with passion for serving the elderly/special needs community.
- Work closely with team players
