If you have a strong desire to help seniors redefine their golden years and continue living vibrantly like we do, join us **You will need to** - Responsible for managing case referrals to support clients' needs - Conduct assessment, planning and intervention for cases by engaging client and building rapport, performing needs assessment and advocating for clients' needs - Develop and review appropriate intervention strategies in consultation - Refer and coordinate care services for clients - Provide information on the various assistance schemes or care options available - Raising awareness and facilitating advance care plan for seniors - Conduct outreach activities to the community **You should** - Thrive in a collaborative and dynamic environment - Have strong communication and stakeholder management skills - Be proficient in Google Suite Applications **It would be great if you** - Have experience in Social WorkAre able to converse in local languages QUALIFICATIONS - Bachelor's Degree preferably in Social Work - Minimum 3 years' experience, preferably in community or healthcare settings in Singapore OTHER INFORMATION This is in partnership with the Employment and Employability Institute Pte Ltd ("e2i"). e2i is the empowering network for workers and employers seeking employment and employability solutions. e2i serves as a bridge between workers and employers, connecting with workers to offer job security through job-matching, career guidance and skills upgrading services, and partnering employers to address their manpower needs through recruitment, training, and job redesign solutions. e2i is a tripartite initiative of the National Trades Union Congress set up to support nation-wide manpower and skills upgrading initiatives.
