**Qualifications**:

- Bachelor of Pharmacy degree from an accredited institution.
- Registered Pharmacist with the Singapore Pharmacy Council.
- 2+ years of experience in direct patient care, preferably in a hospital setting.
- Experience in insulin and warfarin titration.
- Strong understanding of clinical pharmacy principles.
- Excellent communication and interpersonal skills. Ability to work independently and as part of a team.
