**Job ID**:

- We are looking for technically qualified Offshore Surveyor acceptable to Company to execute the Works at site. The personnel shall have at least five (5) full years’ experience in Lifting Equipment Certification for Oil & Gas industry but not limited to the following training and competency:_

**i. Visual Inspection**:

- Wire rope
- Web sling
- Shackles
- Chain sling
- Beam clamp
- Lifting/Pad eye
- Eyebolts
- Hook
- Rigging Screws
- Open Wedge Sockets
- Spreader Bar

**ii. NDT**:

- ASNT Level II Magnetics Particle Inspection (MPI)
- ASNT Level II Dye Penetrant Inspection (DPI)

**iii. Load Test**:

- Pedestal Crane
- Monorail
- Sheave Block
- Jack
- Winches
- Lifting Beam
- Beam Trolley
- Hoists
- Fabricated Pad eye
- Container
- Life Boat

The appointed personnel carrying out the job must be competent, trained and have the required authorization to do the inspection and certification for any particular lifting equipment. Region shall mobilize the suitable competent and qualified personnel for jobs such as inspection of lifting tackles, Inspection/ witness of load test for Pedestal Cranes, Onshore/Offshore load test for hoists, Offshore load test for Lifeboats, winches and etc. Onshore inspection/load test of containers/baskets/special lifting equipment and etc.

**Mobilization Site**
The Mobilization Site shall be at Company Helibase in Kota Bharu, Kelantan, Malaysia.

**Demobilization Site**
The De-Mobilization Site shall be at Company Helibase in Kota Bharu, Kelantan, Malaysia.

**Company’s Helibase and Mustering Point**
Sultan Ismail Petra Airport, Kota Bharu, Kelantan, Malaysia.

**Company Supply Base and Port of Operations**
Kemaman Supply Base (KSB), Kemaman, Terengganu, Malaysia

**Work Site**
Offshore location in Cakerawala Field Development Area of Operations where Works is being conducted or any other location as designated by Company.

The Cakerawala Production Facilities (“Facilities”) which is operated by Carigali Hess Operating Company Sdn. Bhd (Company) is located at Block A-18, approximately 150km NE of Kota Bharu (in northern Peninsular Malaysia) and approximately 100km south of the Bongkot field (in Thai territorial waters) in approximately 55m - 60m of water depth in the Malaysia-Thailand Joint Development Area.

The facilities are equipped with various type of lifting equipment complete with fixed or portable types. Some of the lifting equipment are frequently used for maintenance work especially when it involves lifting of heavy parts, components for change out or to transfer the material from one place to another. In order to ensure the integrity of the lifting equipment and Company’s safety policy has mandatory requirements for the testing and certification of the lifting equipment on regular basis.
