**Responsibilities**:

- Perform health assessment of clients’ conditions
- Carry out individualised planned nursing interventions for the clients
- Assist in procedures and treatments
- Adhere to infection control and prevention practices in client care activities
- Assist clients in their activities of daily living and their instrumental activities of daily living
- Pack medication and provide medication reminder to clients
- Escort and provide support to clients in their daily transport to and fro centre or, for their medical appointments
- Render rehabilitation services according to prescribed exercises and regimen by therapists
- Assist in preparation of meals for clients. (Preparation includes: buying of meals)
- Teach clients, caregivers and family members proper techniques in self-care
- Assist in supporting trained staff in medical emergencies or crisis situations
- Monitor clients for early signs and/ or deterioration using appropriate physiological assessments and observations
- Provide feedback on risk assessment and quality management activities
- Support supervision of support care staff and enrolled nurse trainees in performing basic nursing care activities
- Plan and conduct social, recreational and educational activities to promote clients’ independence, health, wellness, and quality of life
- Guide clients in the development of basic personal and social skills, and simple academics
- Ensure safety of clients at all times by being vigilant and keep environment clutter free
- Perform home care duties such as showering, toileting, changing of diapers and other personal hygiene routine
- Perform laundry and housekeeping duties, eg keeping away sharp objects and maintaining cleanliness of the centre and/client’s home (does not include high rise window cleaning and areas that are unreachable without the aid of a ladder)
- Develop a general knowledge level of MISSION Framework and understanding of AWWA Mission
- Ensure clients’ confidentiality is maintained and adhered to PDPA policy when using systems such as NEHR, and when handling confidential information to stakeholders

**Requirements**:

- Nitec in Nursing and SNB registered Enrolled Nurse
- At least 2 years of nursing experience in acute or ILTC setting
- Passion to work with the elderly
- Able to do Home Care visits
- 5 days work week
