- Instrumental in the implementation of mental health/dementia clinics in primary care
- Collaborate with identified stakeholders to map the referral processes and service linkages to RH, primary care, and community partners for the integration of mental health with physical health
- Conduct ongoing review of primary care model including literature review, outcomes, and learning points
- Key programme manager to work closely with internal and external stakeholders in strengthening the mental health capability plans for primary care partners
- Review and propose recommendations for capability building needs of the primary care providers including working with management and relevant stakeholders in providing the required training in accordance with the stepped care model competencies
- Coordinate and work closely with the identified RH on the plans for training to strengthen the capability of the primary care providers
- Work with service providers on the timely submission of KPIs and claims for processing and reporting while monitoring service provider’s budget utilisation and KPIs
- Work closely with the primary care providers and Regional Health Systems to build the required capability in supporting Healthier SG efforts in going upstream focusing on preventive care and social-health integration
**Job Requirements**:
- Bachelor's degree in any discipline with at least 2 years experience in the healthcare or community sectors.
- Experience in primary care or community sector development and/or multi-agency work in the healthcare/social care industry will be an advantage
- Experience in programme management, implementing new programmes, facilitation, design thinking, and/or knowledge of the healthcare or social industry
- Possess strong analytical, critical thinking, and problem-solving skills with a good eye for detail and strong follow-up skills
- Possess strong organizations skills and effective project management skills in managing multiple projects concurrently
- Strong ability to handle ambiguity and high work pressure in a fast-paced work environment
- Able to demonstrate strong interpersonal skills in handling diverse situations and managing individual differences
