**Locum Medical Social Worker**

We are seeking a dedicated Social Worker to provide essential support to elderly patients and their caregivers. In this role, you will offer psychosocial and financial assistance, along with casework and counselling services to enhance their well-being and integration into the community.

**Responsibilities**:

- Provide psychosocial and financial support to elderly patients and caregivers.
- Conduct casework and counselling sessions to address their emotional and practical needs.
- Collaborate closely with a multi-disciplinary team to ensure comprehensive care.
- Develop and implement coordinated care plans tailored to individual needs.
- Create and manage initiatives and support programs to improve community-based care.

**Requirements**:

- Degree in Social Work and a postgraduate diploma.
- Passion for working with elderly individuals in a healthcare environment.
- Proficiency in vernacular languages to effectively communicate with clients.
- Ability to provide culturally sensitive care and support.

**Duration**:This is a temporary locum position for 4 months, with potential for permanent employment.
