**Roles & Responsibilities**:

- Strengthen Social Work Unit to deepen Person-centred admission to Long Term Residential Care and Community Care.
- Partner collaboratively as a team with multi-disciplinary professionals (Occupational Therapist, Physio-therapist, Therapeutic Programme Executive, Psychologist and Nursing staff ) to innovate, reframe dementia, facilitating holistic person-centred care for persons living with dementia and their caregivers to live a normalised life; empowered and actively engaged in the Lodge and in the community
- Networking with government, hospitals, community, social agencies and community partners to mobilise community resources to support persons living with dementia and cognitive impairments and their families.
- Empower family members living with people with dementia and mild cognitive impairments thru dyadic engagement.
- Contribute as a visionary member in the transformation of dementia care to reframe dementia as an unique phase of life.

**Requirements**:

- Degree in Social Work or Graduate Diploma in Social Work
- Registration with Singapore Association of Social Workers will be an added advantage
- Preferable with minimum 2 years of relevant experience
- Excellent conceptualization, administrative and writing skills
- A visionary, mature, caring, compassionate and pro-active team player
- Possess strong interpersonal skills
- Talents in facilitation of wellness pursuits will be an added advantage.

**Job Type**: Permanent

Pay: $3,500.00 - $4,550.00 per month
