**DESIGNATION**: Programme Coordinator

RESPONSIBILITIES

Plan and conduct on-site activities for seniors that promotes social interaction, and health andwellness in a group setting.

Develop and implement annual calendar plan of social and health activities for seniors

Ensure that the programme and activities meet the performance indicators and operatingguidelines issues by MOH.

To oversee the day-to-day operation and provide supervision to staff and volunteers.

Initiate a case record file for every senior household in the identified block and capture theneeds of the seniors.

Establish and motivate seniors to run mutual help groups; senior volunteers.

Network with other agencies to establish additional avenue of help for the welfare of theseniors

Establish networks with local community-based services or resources to match the need of theseniors.

Oversee and manage the Alert Alarm System (AAS) during the operating hours, renderingassistance when necessary

Maintain an updated register of seniors, volunteers and any other records critical for follow-upand reporting to organisation and relevant authorities.

Conduct home care assessments and complete initial assessments (using Community ScreeningTool) of senior and family to determine home care needs

Plan and conduct on-site activates for seniors that promotes social interaction, and health andwellness in a group setting

Supervise and assist seniors when participating in the activities

QUALIFICATIONS

Passion and desire to work with senior

Self-motivated team player who enjoys working with different profiles of seniors

Able to plan, develop and implement programmes to meet the needs of seniors

Able to engage, befriend and motivate seniors through facilitation of activities or programmes

Able to perform administrative tasks and possess basic Microsoft Office literacy.

Able to prepare and conduct care programmes for seniors; e.g daily news chats/cookingtherapy/art & craft/singing sessions/dancing, reminiscence therapy; etc.

Advance/Higher/Certificate in Community & Social Service (Senior Care Services) and good “O”level (minimum 3 credits) Competency in eldercare service with at least 2 years working experience in an eldercareinstitution. Ability to understand and speak English, Mandarin and local dialects will be an advantageous
