**Job Descriptions**:

- Conduct casework
- Perform stakeholder engagement and partnerships
- Perform community development work
- Design, implement and conduct Groupwork and Interest Based Groups for elderly
- Provide social and emotional support through home visits, care planning and activity recommendation
- Provide support for volunteer management
- Maintain and document records of groupwork and case sessions
- Provide support to Case Managers
- Coordinate, plan, develop, implement and evaluate programmes
- Conduct environmental scans and community need assessments

Tembusu Active Ageing Centre and Tembusu Eldercare Centre are located at Eunos Crescent.
