Assistant Senior Social Worker, Aftercare Programme

Responsible for designing, implementing, and evaluating the care leavers’ individual care plans, interventions, and community integration efforts. Ensuring that the individual care plans, interventions, and community integration efforts are trauma-informed and in accordance with local professional ethics and standards and prevailing international standards.
Provide supervision to caseworkers and/or social workers in Melrose Home, ensuring that the caseworkers/social workers’ processes and practice standards are in accordance with MSF’s licensing requirements, local professional ethics and standards, and prevailing international standards.
**Duties and Responsibilities**
Leadership and Supervision
The incumbent is to provide leadership in the following areas:
- To oversee case planning, case management, therapeutic interventions, admission/discharge processes, and case management processes of Care Leavers
- To ensure that the case planning, case management, and therapeutic interventions align with professional standards and duty of care.
- To ensure that the interventions are trauma-informed and meet MSF’s philosophy of care and other standards set out within CAS and/or by international professional bodies guiding residential care.
- To provide supervision, guidance, and leadership to caseworkers/social workers, ensuring that the caseworkers/social workers’ processes and practice standards are in accordance with MSF’s licensing requirements, local professional ethics and standards, and prevailing international standards.
- To support the Manager of Social Care in the review and implementation of SOPs in Case Management
- To identify and assess risks and strengths inherent in care leavers by integrating a range of information.
- To use professional tools and working templates in case assessment and case planning
- To ensure the effective and efficient implementation of care plans
- To represent the youth (either during case conferences or engagements with stakeholders), to advocate for these children, youth, and families to have access to equal opportunities through collaborative practice with community partners
**Groupwork and Other Programmes**
- To participate in client/community assessments, group/community work processes, and specialized intervention strategies
- To engage Care Leavers in relevant programmes to help support the youth’s potential and optimize their transition to independent living.
**Advocacy**
- To represent Melrose Home in its advocacy efforts for vulnerable children and youth and their families, to advocate for these children, youth, and families to have access to equal opportunities through collaborative practice with community partners.
- To represent CAS and the Home at professional platforms to raise the profile of the Society and the Home and also to build professional relationships that bring visibility and increase the Home and Society’s standing in the field
**Administration (Operational Processes, Policies and Protocols)**
- Comply with the Case Management Standard Operating Procedures set by Melrose Home
- Undertake any other duties directed by the Director of Home.
**Qualifications and Requirements**:
- Degree in human-related services (preferably in Social Work or Psychology)
- Minimum 6 years of relevant experience in the social service sector and 2 years in a supervisory position (relevant experience in a residential care environment would be useful).
