- Overview
The Senior Social Worker will be responsible for case management and providing care provision and formulation of Care Plan for the residents of Home.- Job Description
Casework management and counselling services of residents of homes and family members of residents
-Development of individual care plans of residents
-Provide guidance and clinical supervision to junior social workers
-Review interventions, case progress and determine case closure
-Ensure compliance with regulatory requirement and internal procedures by the Management and relevant government agencies
-Plan and conduct group work and/or community work for residents
-Work closely with multi-disciplinary team or agencies to deliver care and case plans
-Prepare statistics reports for management review
-Ensure documentations are in place
-Any other duties as assigned by the Head of Home.
- Job Requirements
Minimum Degree in Social Work / Psychology / Sociology
-At least 5 - 6 years of casework and counselling experience
-Advantage for those who are certified in doing mandatory counselling cases
-Good writing skills, interpersonal and communication skills
-Meticulous and a team player

-Alt Saturday: 9.00am - 1.00pm
