**STATEMENT OF PURPOSE**
Responsible for supporting the multidisciplinary team in the Specialist Outpatient Clinic in processes related to the coordination of care for patients.
**MAJOR DUTIES AND RESPONSIBILITIES**:
**(A) SPECIFIC (80%)**
- Provide administrative and coordination support to care coordinators, social workers and nurses in management of patients in the Specialist Outpatient Clinic (SOC):
Conduct assessment of patients/caregivers using relevant assessment surveys
- Liaise with care team, patients and their families to initiate and coordinate appropriate referrals to internal departments and external parties e.g. community partners.
- Scheduling of appointments and liaise with Customer Contact Centre and respective SOC, where needed.
- Engage and follow-up with patients and their families to gather relevant information required to put up referrals for services, schemes or financial assistance, where needed and when necessary.
- Work closely with the multidisciplinary team to support the coordination of care for identified patients in SOC.
- Ensure timely follow up with patients/caregivers on plans or activities as required by the multidisciplinary team, including the documentation of such communications with patients and their families.
- Provide operational administrative support to the multidisciplinary team where needed, including data extraction and data processing.
- **GENERAL (20%)**
- Embrace continuous improvement by participating in improvement initiatives.
- Maintain electronic information record of patient as established by organization.
- Carry out other duties as assigned by the supervisor or Head of Department.

**JOB REQUIREMENTS**:
**(A) EDUCATION, TRAINING AND EXPERIENCE**
- Higher NITEC and above
- Minimum of 2 years in service industry/health-care related areas preferred
- Possess positive attitude, initiative and keenness to learn.
- Possess good interpersonal and communication skills
**(B) SPECIALISED TRAINING**
- Knowledge of Excel
