**We’re Hiring: Case Manager**

**Location**:Buangkok Green

If so, we invite you to join our dedicated team as a **Case Manager**in a **psychiatric welfare home for destitute male adults**. This is more than a job — it’s a calling.

**What You’ll Do**

**1. Case Management & Holistic Care**
- Conduct thorough bio-psycho-social-spiritual assessments
- Develop and review Individualised Care Plans (ICPs)
- Coordinate with nursing, allied health, and operations for person-centered care
- Liaise with government and community partners (e.g., MSF, AIC, IMH, HDB)

**2. Psychosocial Support**
- Provide psychoeducation and basic counselling
- Facilitate therapeutic activities and social skills groupwork
- De-escalate conflicts and intervene during crisis situations
- Identify and support residents at risk of self-harm, aggression, or isolation

**3. Discharge Planning & Reintegration**
- Engage residents and families in discharge preparation
- Source suitable step-down and community-based care options
- Conduct home visits to assess post-discharge readiness

**4. Advocacy & Family Engagement**
- Liaise with external agencies to advocate for resident welfare
- Engage families to strengthen support systems
- Recommend policy improvements based on ground experience

**5. Documentation & Compliance**
- Maintain accurate and timely case notes
- Prepare reports for audits and case conferencesEnsure compliance with PDPA and internal SOPs

**Who We’re Looking For**
- Degree or diploma in **Social Work**, **Psychology**, **Counselling**, or a related field
- Fresh graduates with relevant experience are welcome
- Sound knowledge of **mental health**, **rehabilitation**, and **community resources**:

- Strong in crisis intervention, communication, and teamwork
- Able to work independently and handle emotionally demanding situations
- Willingness to work **in a male psychiatric setting**.
