The Social Worker & Outreach Assistant Manager will provide support and interventions to persons with disabilities at the E-Commerce Fulfilment Training Centre. He/She is committed and service
- oriented and works in collaboration with other agencies and partners to reach out and provide support to persons with disabilities in the provision of training and job placement support. He/She will work closely with other Allied Health Professionals in supporting persons with disabilities towards gainful employment.
**Responsibilities**:
- Conduct needs and intake assessments
- Perform case planning and goal setting
- Implement case plans and interventions
- Review casework interventions for improvements
- Conduct closure of cases upon achieving sustainable service outcomes for clients
- Maintain case notes, documents and prepare social reports
- Plan, facilitate and evaluate group work interventions
- Support, implement and evaluate employment-related development initiatives
- Administer pre and post clients’ survey
- Interview clients and create client profiles for in-house marketing purposes.
**Outreach**
- Be engage in outreach events and activities to secure partnerships and referrals of clients and identify
placement opportunities with (SSA’s / SPED’s / IHL’s)
- Conduct learning journeys to visiting orgs/corps
**Requirements**:
- Degree in Social Work or equivalent
- Able to work well with team members to deliver employment outcomes for clients
- Passionate in working with persons with disabilities
- Possess good knowledge & experiences with social work interventions and social policies
- Competent in administration and organisation of case notes and recordings
Salary range: $3820 to $4500
