**Case Worker**

A secular and non-ethnic Community Services organisation, our client has a vision to serve all people regardless of race or religion, with a heart. It is a full member of the National Council of Social Service (NCSS)

They are now seeking a Case Worker to join their residential care home in the North East of Singapore.

Key responsibilities:

- Assess and develop individualised care plans for the residents
- Assist in the development and implementation of programmes for the residents through data collection and the conduct of basic analysis
- Coordinate care for the residents with relevant stakeholders
- Documentation of case notes
- Facilitate timely and appropriate service referral and provision to residents
- Participate in case discussions with external service providers
- Provide case management services
- Perform needs assessment of seniors living in the community
- Prepare data for submission to relevant authorities

The Key Technical Competencies we are seeking include:

- Degree in Social Work or Psychology
- Able to engage stakeholders to develop solutions in community work
- Able to work independently and in a multidisciplinary team.
- Good interpersonal and communication (oral and written skills)
- Ability to work rotating shifts
